• Minor – Under 18 years of age (Valid for entire 2011/12 Winter Season)
• Print Parent Name on Left / Sign Parent Name in Middle / Print Child Name/Age on Right

• Minor – Under 18 years of age (Event Specific)
• Print Parent Name on Left /Sign Parent Name in Middle /Print Child Name/Age on Right

• For Minor: Sign Parent Name on Left /Print Child Name on Right on Page 1
• For Minor: Sign Parent Name on Left /Date on Right on Page 2
• Must check "I Consent" to medical treatment or athlete will not be allowed on course
• Must check "I certify I am the Parent / Legal Guardian"
• FILL OUT COMPLETELY!
